Enhanced Living now offering the Transition Home Program

Though similar to a traditional respite care, there are notable differences. Most often prearranged respite has been utilized to offer a break for caregivers going on vacation or contending with their own health issues. In most cases respite clients were in stable situations at home and did not need ancillary services above and beyond standard personal care services during their short stay with us. Transition Home clients often need continual therapy and other ancillary services to ensure a safe return home. The number one goal of Transition Home is to get clients back home as safely and efficiently as possible by helping individuals regain and maintain their independence. Below are the services and amenities available based on individual need.

24 hour emergency call system
Three chef-prepared meals a day
Group transportation
Access to basic Enhanced Living services, amenities, and activities
Assistance with activities of daily living
Medication monitoring and administration
Laundry, linen service, and housekeeping
Social Services
Basic colostomy/incontinence care*
Oxygen therapy*
Wound care¹*
Physical, Occupational, and Speech Therapy evaluation and treatment¹*
Safety training and energy conservation for cooking, laundry and household chores¹*
Safety training for tub and shower transfers¹*
Safety training for vehicle transfers¹*
Home safety and functionality assessment¹*
Home modification recommendations and assistance with medical equipment needs¹*
Interdisciplinary discharge planning*
¹ Provided by AdvantageCare Rehabilitation, LLC and/or Advantage Home Health Services based on evaluation and insurance coverage. These services are available for Respite with a medical recommendation. Copays and fees may apply.
