Wagner Transitional Care
Wagner Transitional Care features private rehab rooms with private bathroom accommodations. It is designed for individuals who are recovering a variety of injuries and illnesses, including stroke, hip and knee replacements, degenerative neurological and orthopedic disorders and impairments that require rehabilitative services including physical, occupational, and speech therapies as ordered by a physician.
Our interdisciplinary team of caring and experienced therapists create
individualized treatment plans based on your abilities and goals for recovery.
They treat you with kindness and respect while promoting maximum health
and restoration, while getting you back on the road to recovery and back to functioning at the highest level possible.

Memory Care
The Asper Memory Care neighborhood offers specialized services for those with a dementia diagnosis such as Alzheimer’s disease who also require nursing care services.
A safe and secure environment promotes individuality and dignity and offers group and individualized programs that maximize independence and provides appropriate stimulation. Most notably our Music Therapy Program brings a board-certified music therapist to offer personalized, clinical service to improve outcomes, behaviors, and holistic wellness for residents. This formalized music therapy program takes music beyond art form and entertainment and helps residents creatively express their memories and feelings.
Additionally, an exciting new technology called the Mobii Magic Surface projector has proven to be a great addition to our nursing neighborhoods. It’s innovative programming is especially beneficial for residents in memory care as it simulates a wide variety of familiar activities through projected images that you can interact with through simple movements. The product has won several awards related to Dementia Care and we’re very glad to be one of less than 50 users of Mobii in the United States.
